Important note on appointment confirmation
Your appointment for the health check will only final and binding bookingas soon as we receive the following documents completed and signed received from you:
✔ Medical questionnaire
✔ Patient consent
Please send us these documents at least 24 hours before your appointment This simplifies the on-site procedures and ensures that your examination is optimally prepared.
Without these documents, the appointment cannot be reserved bindingly. You will receive the forms as PDFs to your email address after booking your appointment with Doctolib.
